- Review current supports and services
- Explain Adult Autism Waiver services and expectations
- Identify immediate needs or concerns
- Guide individuals through next steps in the coordination process
Person-Centered Planning and ISP Support
Deborah Care supports the development and ongoing maintenance of the Individual Support Plan (ISP). Our role includes:
- Supporting person-centered planning discussions
- Ensuring the individual’s voice and choices are central to the plan
- Identifying services and supports that align with goals
- Assisting with ISP updates when needs, circumstances, or preferences change
Coordination of Services and Providers
We help individuals connect with and coordinate services across multiple providers by:
- Supporting informed choice of qualified providers
- Facilitating communication between providers, individuals, and teams
- Helping resolve service delivery concerns
- Ensuring services align with the approved ISP
Ongoing Monitoring and Follow-Up
Support Coordination is an ongoing service. Deborah Care provides:
- Regular check-ins with individuals and providers
- Monitoring of service effectiveness and satisfaction
- Assistance addressing changes in health, housing, or life circumstances
- Support with problem-solving and service adjustments
Advocacy and Resource Connection
We assist individuals with:
- Understanding their rights and responsibilities
- Accessing community resources and natural supports
- Navigating challenges related to services or supports
- Identifying alternatives when services are not meeting needs

- Enrolled in or seeking services through the Adult Autism Waiver
- In need of Support Coordination services
- Looking for a collaborative, respectful, and responsive Support Coordination provider

We gather information about your needs, goals, and current services to understand how we can best support you.
Step 3: Planning and Coordination
We support person-centered planning and help coordinate services and providers according to your Individual Support Plan.
Step 4: Ongoing Support
We maintain regular communication, monitor services, and assist with changes or concerns as they arise.
Accessibility and Communication Supports
Deborah Care is committed to ensuring meaningful access to services for all individuals.
Communication Assistance
We will arrange appropriate communication supports for individuals who are:
- Deaf or hard of hearing
- Blind or visually impaired
- In need of alternative communication methods
Supports will be provided based on the individual’s needs and preferences whenever possible.
Language Assistance (Limited English Proficiency)
Free language assistance services are available to individuals with Limited English Proficiency (LEP). These services may include:
- Interpretation services
- Assistance understanding written or verbal information
There is no cost to the individual for language assistance.
